first appointment
For your first appointment, please bring
- Driver’s license or other government issue ID
- Medical insurance card or cards
- Medication list with dosage and frequency
- Any past medical history or medical records you would like reviewed
- If you have imaging (CT scan, MRI, X-Ray, Ultrasound) you would like reviewed; please bring both the radiologist report and a disk containing the imaging study
- For patients who have a guardian with power of attorney, please bring the power of attorney documents and the person with power of attorney so consent for care and treatment can be obtained
Please arrive at least 15 minutes prior to your scheduled appointment to complete necessary paperwork.
